Our Complaints Process

Every complaint follows a structured four-stage process. You will receive a reference number at Stage 1 and can use it to track progress at any time.

We receive your complaint by WhatsApp, email, phone, or in person. You are issued a unique complaint reference number and a named case handler is assigned.

Investigate

Your case handler reviews all relevant records - dispensing logs, delivery history, consultation records, payment data. For clinical complaints, our pharmacist-in-charge or medical officer is involved.

Respond & Resolve

We contact you with our findings, a clear explanation of what happened, and the remedy we are offering. We aim to resolve all complaints at this stage. If more time is needed, we tell you why and when you can expect a full response.

Close & Learn

Once resolved, your complaint is closed and recorded in our quality log. All complaints are reviewed monthly by leadership to identify patterns, improve systems, and prevent recurrence.

What We Will Do

Remedies We Offer

When we get something wrong, our response is proportionate to the seriousness of the failure. We do not offer token apologies - we fix the problem.

We Always Accept Responsibility When We Are Wrong
We will never deflect, dismiss, or minimise a genuine complaint. If we made an error, we say so clearly, explain what happened, and take concrete steps to fix it.
🔄 Replace or refund - For wrong or damaged medicines: immediate replacement at no cost or a full refund, whichever you prefer.
💸 Billing corrections - Overcharges are refunded within 3 working days of the complaint being upheld.
🩺 Clinical review - If clinical advice was incorrect, a senior pharmacist or medical officer reviews and corrects the guidance in writing.
👤 Staff accountability - Staff behaviour complaints are investigated and appropriate action taken, including retraining, formal warning, or escalation to management.
⚙️ System changes - Where a complaint reveals a process gap, we change the process - and tell you what we changed and when.
🤝 Sincere apology - A meaningful, personalised apology from the relevant team lead or director - not a copy-paste template.

Adverse Drug Reactions & Pharmacovigilance Reporting

As a PCN-licensed pharmacy, HubPharm has a legal and ethical obligation under NAFDAC's pharmacovigilance programme to report all suspected adverse drug reactions (ADRs). If you or someone in your care has experienced an unexpected or severe reaction to a medicine dispensed by HubPharm, report it to us immediately.

  • All ADR reports are treated as urgent clinical cases - not standard complaints
  • You will be contacted by a senior clinical pharmacist within 2 hours of the report
  • We will document the reaction in full and report to NAFDAC's Yellow Card system within the required timeframe
  • If you need emergency medical help, call 112 or go to your nearest hospital immediately - do not wait for us to respond
  • Your ADR report helps protect other patients across Nigeria - we take every report seriously
📋 What to Include in Your ADR Report
The name and dose of the medicine. The reaction you experienced and when it started. Any other medicines you are taking. Your order number or prescription reference if available.

You can also report adverse reactions directly to NAFDAC's pharmacovigilance unit. HubPharm reports all ADRs on your behalf, but you always have the right to report independently.

🔄 Medicine Quality Defects
If you believe a medicine is substandard, falsified, or has a manufacturing defect - regardless of whether you experienced a reaction - report this to us immediately. We will quarantine the batch and report to NAFDAC.

Escalation Pathway

If you are not satisfied with how HubPharm has handled your complaint, or with the outcome, you have the right to escalate to our regulatory bodies. We support your right to do this without restriction.

The PCN is Nigeria's pharmacy regulatory body and has authority to investigate complaints about licensed pharmacies and pharmacists. You may file a complaint if you believe our professional standards or licensing obligations have been breached.

Nothing in this complaints policy limits or removes your right to pursue legal remedies. This policy governs our internal complaints process only. You retain full legal rights under Nigerian law regardless of the outcome of our internal process.

Yes. You may raise a complaint on behalf of a family member, patient, or person in your care. Please state clearly at the start of your complaint that you are raising it on behalf of another person, provide their name and your relationship to them, and confirm that they have given consent for you to do so. For deceased patients, next of kin may raise complaints with appropriate documentation.
Absolutely not. Your access to medicines, the quality of your pharmaceutical care, and your standing as a HubPharm patient will not be affected in any way by raising a complaint. We take a zero-tolerance approach to any form of retaliation against a complainant. If you ever feel that your care has been affected after raising a complaint, please escalate this to senior management immediately.
All complaint records - including correspondence, investigation notes, and outcomes - are retained for 6 years in line with pharmacy regulatory requirements and the Nigeria Data Protection Act 2023. Complaint records are stored securely and are accessible only to authorised personnel involved in quality assurance and regulatory compliance.
If you are not satisfied with the outcome of your complaint, you may request a senior management review by emailing care@hubpharmafrica.com with "Escalation Request" and your complaint reference number in the subject line. If you remain dissatisfied after that review, you have the right to escalate to the Pharmacy Council of Nigeria (PCN), NAFDAC, the Nigeria Data Protection Commission (NDPC), or the FCCPC depending on the nature of your complaint. We support your right to escalate without restriction.
Yes. If Afiya AI provided you with health information that was incorrect, misleading, or potentially harmful, we want to know. Complaints about Afiya AI are reviewed by our clinical pharmacist team to assess whether the information was clinically accurate and appropriate. Where errors are found, we update Afiya's training data and document the correction. Afiya AI is a support tool - it does not replace professional medical advice - but we still hold it to a high standard of accuracy.
