What blood tests do I need while taking palbociclib?

A full blood count is required before starting treatment, at the beginning of each cycle, and on day 15 of the first two cycles. Neutropenia is the most common side effect and your oncologist may delay a cycle or reduce the dose based on these results.

What are the common side effects of palbociclib?

The most frequently reported are a low neutrophil count, infections, tiredness, nausea, mouth sores, hair thinning, diarrhoea, low red blood cell count and low platelet count. Report fever, sore throat or any sign of infection to your oncology team urgently.

Can I eat grapefruit while taking Ibrance?

No. Avoid grapefruit and grapefruit juice throughout treatment, as they can raise palbociclib levels in the blood and increase side effects.

What should I do if I miss a dose or vomit after taking it?

How Palbociclib Works

In hormone receptor positive breast cancer, oestrogen signalling drives the cyclin D-CDK4/6 pathway, which phosphorylates the retinoblastoma protein and allows the cell to move from G1 into S phase and divide. Palbociclib inhibits CDK4 and CDK6, holding cells in G1 and blocking that division. Combining it with endocrine therapy attacks the same pathway at two points, which is why the two are always given together.

Monitoring

Full blood count before starting, at the start of every cycle, on day 15 of cycles 1 and 2, and whenever clinically indicated. Treatment is normally withheld if the absolute neutrophil count falls below the threshold your oncologist sets, and resumed at a reduced dose once counts recover.

Storage

Store below 30°C in the original pack, out of the sight and reach of children. Do not use after the expiry date on the carton. Return unused oncology medicine to a pharmacy for safe disposal.
